We are experiencing some issues with the ‘send a confirmation email to’ field which may mean that you may not receive a confirmation email or that you receive an error when submitting the form. Please be assured that we will still receive your registration / referral, there is no need to contact us to check unless you cannot submit the form entirely.
Explanation of terms
Please read all the options below before submitting the form to ensure that we allocate your case accordingly, allowing you to receive the best service from us.
Please note that a What Matters Conversation (Carer Needs Assessment) is required before specific services can be offered, services are offered based on needs identified in an assessment and that some of our services and resources are subject to funding and availability.
You will be registered on our system and be able to contact us at any time if you require support or wish to access our services. Please note if you select this option we will not contact you. You will be kept up to date with the latest news via our quarterly newsletter and emails if you opt in to receive it.
You wish to register with NEWCIS and would like us to contact you back to see how we can best support you.
Please provide us with as much information about your caring role and what you would most like help with at the bottom of the form so that we can allocate your case accordingly.
We aim to be in touch within 7-10 working days.
Professionals, family members and friends can refer an unpaid carer to us.
If you are a professional, please provide as much detail about current support and services that the carer is currently open to.
Please ensure you have consent from the person you are referring to us as we will need to contact them.
Under 18 - Referrals will need to be made using the Young Carers referral form
These will be dealt with by the Young Carers team directly. Once allocated, one of the team will contact you.
Over 18 - Referrals will need to be made using the NEWCIS adult service referral form (below)
This is because the adult team at NEWCIS will deal with the referral, undertake any assessments needed and provide support. Once you have been allocated to a worker to complete an assessment please ask them to forward your details to the Young Carer team, if you would like to access the group, so that we can add you to our contact list.
The personal details you provide will be stored by NEWCIS on a secure database in accordance with the General Data Protection Regulations.
