PURPOSE OF ROLE:
To provide a comprehensive service for carers. Offering them information, advice, signposting, assistance and assessment. Empower carers to understand their rights and be supported with their own well-being as laid out through the framework of the Social Services and Well-being (Wales) Act 2014.
Provision of direct support to carers accessing community hospitals in the Denbighshire area and within the acute hospital located in Bodelwyddan. Working with BCUHB(NHS) staff to identify carers and ensure they are supported. To assist with prevention of carer breakdown and help facilitate timely/safe discharge from hospital of loved ones and prevent re-admittance.
Facilitation of assessments, which adopts a strength based holistic approach to a carers life and circumstances. Aim to empower and support the carer with achieving their own personal well-being outcomes. This service will be delivered in a person-centred way through formal and informal methods.
You will need to work as part of the wider NEWCIS team which will include inclusion on a duty rota, carrying out home visits to carers and assisting with activities and fundraising events. You will need to be able to use NEWCIS’ data base, the local authority’s data base and email systems to ensure all monitoring processes are adhered to.
MAIN DUTIES AND RESPONSIBILITIES:
Assist Community & Acute Hospitals in Denbighshire to support carers and continue to raise awareness of carer issues within Community/Acute Hospitals.
Engage with Hospitals/BCUHB staff both day to day and during ward rounds/MDT meetings to identify and support carers.
Provide information, advice and signposting on a range of services and community support available to the carer, linking to the resources of DEWIS, NEWCIS and SPoA.
To inform all carers of their entitlements under the SSWB Act, including their right to an assessment of their needs and encourage them to take this up as a tool to support them.
Undertake What Matters/Carer needs assessment for carers either within the hospital setting, carers home or NEWCIS offices.
Adopt the ‘what matters’ approach to assessments, which is a strength based holistic assessment that empowers and supports carers to achieve their well-being outcomes.
Encourage Carers who are looking after someone to register with NEWCIS which will enable them to receive long term support.
Support carers with the discharge process from hospital to home informing of support and signposting to relevant services.
Inform carers of the NEWCIS training, counselling, group activities and volunteering opportunities for carers and past carers to enable them to receive peer support whilst developing their skills and confidence.
Be part of the Duty Rota System as required answering calls/emails that are received from carers.
Participate in collaborative events with statutory services, voluntary and private sector to increase understanding of what maintains carers health and wellbeing.
Facilitate opportunities for carers to meet and receive support to help them understand and cope with the loss of a loved one – where appropriate
Ensure regular monitoring and review of the service’s activity, together with the collection of evidence / data to enable the evaluation of the service outcomes/contract obligations. This will include adhering to the quality standards required by Charity Excellence, reporting and providing regular updates on progress of the work to the Team Leader in accordance with reporting requirements.
Maintaining and ensuring confidentiality over personal information relating to individual users of NEWCIS and company information as per the General Data Protection Regulation 2018.
It is the general duty of all employees to assist with Health and Safety processes and monitoring in the NEWCIS Carers Centre on a regular basis and assist with key holder duties.
To undertake any other duties as reasonably requested by management team of NEWCIS.
COMMUNICATIONS:
Communication with carers is an essential part of the post to ensure good customer service is achieved. This includes updating the NEWCIS data base ensuring that relevant staff can access data in order to support carers effectively and adhere to legal requirements on recording and data handling.
Work with the team leader and staff in the co-ordination and production of communications, including digital, newsletters, posters, advice literature, press releases, social media and publicity materials as required by the post when facilitating services for carers.
Work with the administration team to co-ordinate the ordering and supply of communications and PR materials, liaising with the Administration Supervisor.
GENERAL:
Teamwork is essential to the smooth running of the organisation and all members are required to play an active part. Staff duties include helping to maintain cover in the office, dealing with calls from carers and following all the policies and procedures of the organisation. Everyone has a responsibility to work co-operatively and in collaboration with all members to ensure that the organisation meets the requirements of the service level agreements it has entered into.
Due to the nature of the job and organisation, some evening and weekend work will be necessary for which time off in lieu can be taken.
LOCATION:
The role is based at the Carer Centre in Mold / Denbighshire acute (Glan Clwyd) and community hospitals and with some home working. As such a current UK driving licence and access to a vehicle or access to equivalent transport is required.
This job description sets out the main duties of the role as at the date it was drawn up. It is expected, therefore, that the jobholder will undertake any other duties commensurate with the grading of the post as allocated through line management.

RISK MANAGEMENT:
It is a standard element of the role and responsibility of all staff within the Charity to fulfil a proactive role towards the management of risk in all their actions. This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards.

All employees of the Charity have a statutory duty of care for their own personal safety and that of others who may be affected by their acts or omissions.
